Appointments


Appointments are scheduled between 9 a.m. and 3 p.m. Monday-Friday (except KU/federal holidays).

Please allow 4-6 weeks to receive your passports. Printable applications are available at www.travel.state.gov or get a hard copy at International Programs.

Things to bring to passport appointment

  1. Passport Application Form DS-11 (unsigned) or DS-82 for renewals

  2. Proof of U.S. citizenship (one of the following)
    • Previous valid U.S. passport
    • Certified U.S. birth certificate
    • Consular report of Birth Abroad or Certification of birth

  3. Proof of Identity (one of the following)
    • Recently issued Certificate of Naturalization or Citizenship with easily recognizable photo
    • Current, valid driver's license
    • Government employee ID
    • Military ID

    For children under age 16, the child and both parents or legal guardian(s) must appear in person and present the following:
    • Evidence of the child's U.S. citizenship
    • Evidence of the child's relationship to parents/guardian(s) AND
    • Parental/guardian identification

    If only one parent appears, you must also submit one of the following:
    • Second parent's notarized written statement consenting to issuance for the child
    • Primary evidence of sole authority to apply
    • A written statement (made under penalty of perjury) explaining the second parent's unavailability.

  4. One passport photo (can be taken at International Programs)
    • 2x2 inches in size
    • Clear likeness, taken in the last six months
    • Color
    • Full-face, front view on white or off-white background
    • Normal street attire, no hats or headgear

  5. Passport fees (see below)

$15 for two photos (can be used for passport or visa application and for other official documents)

Fees


Passport book - $110

Passport card - $30 (valid only for sea or land entry to and from the Caribbean, Canada, Bermuda, and Mexico)

Expediting fee - $60 (for rushed application processing)

The above fees must be paid for with check or money order made out to U.S Department of State .

Processing fee - $25

Photos -$15

The above fees must be paid for with cash, check, or money order made payable to "University of Kansas".

Overnight delivery of application and return of passport are additional fees.
